Card or Visa.
Insurance:
With your permission,
I can electronically submit insurance claims on your behalf. I can also provide you with a "Super Bill" that you
can submit to your insurance company to receive reimbursement according to your plan. If your insurance covers "out-of-network"
providers, you may receive reimbursement of up to 60 or 70% of the session fee.
I highly suggest calling your insurance company to ask what your mental health benefits are, and if you have a deductible
to be met.
Fees:
My fee is $125.00 per 50 minute session, however, in some cases and when there are openings, I am able to offer a
sliding scale fee based on your financial status.
